To complete your DOPP enrollment, please select one of two enrollment paths:
Healthcare Enrollment or Non-Healthcare Enrollment.
Choose the Healthcare path if you are enrolling through a healthcare provider, clinic, hospital, or medical program.
Choose the Non-Healthcare path if you are enrolling independently or through a non-medical program.
Please note that one must complete the form fully for the application to go through.
